1.     The OO Eye Exerciser Device with a power supply adapter.

Here comes the rescue with the Sitting and Reading/Writing Position Monitor/Beeper, which is small, light, and operating on a small watch battery that lasts for months/years.

Put the monitor on an ear; adjust the angle properly when the kid is reading, writing, or doing any close-up work.   The monitor will beep that can only be heard by the kid him/herself when he/she lowers head to certain position.
